Available for download below are the entry forms for the various tournaments throughout the season. Just click on them and they'll automatically start downloading. If you do not have Acrobat Reader click on the link below and your browser will be directed to the Adobe website where it can be found.
1. Please
print your name on the form and
mail early to insure a spot to play.
2. Be sure the form is complete and your dues are paid, it reduces the number of telephone calls.
3. If you have scissors or a paper cutter
please separate multiple entries. No staples please.
4. You may combine multiple entries and pay with just one check.
If you enter and then find you can't play call 624-4758 (Mel Hamilton) for a
refund.

When you take your
personal golf cart please volunteer the
trail fee. If you are
posting your
score on GHIN post as a tournament score by selecting
type.

All cash winnings and certificates will be distributed by mail twice yearly at the end of each half of tournaments. You may use certificates to pay for tournament entries and optional $5 games (certificates = good as cash).
